Connections Memory Café

Who: Individuals living with cognitive changes and mild memory loss PLUS their caregivers.

What: Come enjoy a fun time out and about, enjoying time together away from the stresses at home. We will have lunch, games, and activities for you to enjoy.

When: Monthly. Check the activities calendar for details.

Chat to Heal: A Caregiver Group Support Program

Who: Caregivers caring for people living with cognitive changes and memory loss. 

What: Join us for an hour-long, facilitated support group. Talk, support, and share resources with people who are living with the same challenges as you are. Know that there are people and tools to help. Facilitated by Deana Fisher, LMSW, Certified Montessori Dementia Care Professional. Respite care is available for your loved one.

When: Every other Wednesday.  Check the activities calendar for details.

Brain & Body Wellness Workshops

Who: People who have concerns about their own brain and/or memory changes and people living with mild dementia.

What: Educational group where people learn, connect with others, and practice techniques to strengthen brain, body, creativity, sense of humor, and quality of life.

When: 6-week workshop sessions held 4 to 5 times each year.  Check the activities calendar for details.

Memory Support Programs are funded by 5 Healthy Towns Foundation.

The 5 Healthy Towns Foundation (5HF) – formerly Chelsea-Area Wellness Foundation – is a tax-exempt, private foundation governed by a volunteer board of directors representing populations included in the school districts of Chelsea, Dexter, Grass Lake, Manchester, and Stockbridge, generally referred to as the 5HF service area. The mission of the 5HF is to cultivate improvements in personal and community wellness.

5 Healthy Towns Foundation

Funding for all CSC programs comes significantly from local support, including sponsorships and private giving, as well as from grants. Every dollar impacts seniors in our community every day.
